Hospice Liaison (Business Development) – RN Preferred
Job Description
Based in Racine, Wisconsin, Lily Home and Hospice is seeking a Hospice Liaison with RN preference to join its on-site team. This role focuses on building durable referral networks and educating healthcare professionals and families about hospice services, with a clear emphasis on driving growth within the community.
Responsibilities
- Develop and maintain strong referral relationships with hospitals, physicians, SNFs, ALFs, and community partners
- Educate healthcare professionals and families about hospice services
- Serve as a trusted resource and advocate for patients transitioning to hospice care
- Collaborate closely with clinical and leadership teams to support growth initiatives
Requirements
- RN preferred
- Experience in hospice, home health, healthcare sales, or business development strongly preferred
- Excellent communication and relationship-building skills
- Compassionate, self-motivated, and goal-oriented
