Hospice Liaison (Business Development)

Lily Home and Hospice is hiring a Hospice Liaison (Business Development) to strengthen referral pathways and educate clinicians and families about hospice care in the Racine, WI area.

Responsibilities

• Build and maintain referral relationships with hospitals, physicians, SNFs, ALFs, and other community partners
• Educate healthcare professionals and families about hospice services
• Act as a trusted resource for patients during the transition to hospice care
• Advocate for patients as they move into hospice services
• Collaborate with clinical and leadership teams to support and advance growth initiatives

Requirements

• Experience in hospice, home health, healthcare sales, or business development (strongly preferred)
• Strong communication skills and the ability to build relationships
• Compassionate approach, with self-motivation and a goal-oriented work style

Compensation

• $70,000 - $80,000 per year
