Hospice Business Development Liaison - RN

Join By the Bay Health as a Hospice Provider Relations RN, a role dedicated to building steady partnerships with referral sources and guiding patient admissions for a local nonprofit hospice serving Marin and San Francisco Counties, with additional coverage to Sonoma County as needed. This on-site position centers on coordinating care from initial referral through admission, ensuring smooth transitions for patients entering the hospice program.

Responsibilities

• Develop relationships, maintain regular contact, and resolve issues with referral sources including physicians, hospitals, nursing facilities, residential care facilities for the elderly, senior housing, and community service agencies to ensure ongoing quality of care and referrals
• Determine patient's eligibility for hospice care
• Admit patients, obtain informed consent(s) for service, inform patient/family of rights and responsibilities
• Assist with the implementation and coordination of services required for the discharge of patients from hospitals into agency's hospice program
• Communicate with Intake and Admissions department regarding hospice referral/admission
• Assist Director of Provider Relations in negotiating or redefining contract provisions as necessary and in accordance with relevant laws and regulations

Requirements

• AA or B.S. degree in Nursing from an accredited Nursing School
• Minimum of one experience in a home health/ hospice setting
• Current license as a registered professional nurse in the State of California

Wednesday - Monday; 8:30am - 5:00pm

Location

Marin & San Francisco Counties with additional coverage to Sonoma County as needed

Pay Range

$65.71 - $75.70 hourly
