Business Development Liaison – Home Health

Grace Home Health is hiring a Business Development Liaison to strengthen referral partnerships and drive steady Traditional Medicare home health admissions in Rockwall County and nearby communities. This field-based role focuses on strategic outreach, education, and ongoing face-to-face engagement with key healthcare referral sources.

The successful candidate will build and maintain relationships across the local continuum of care while supporting monthly admission growth. You will coordinate closely with intake and clinical leadership to help ensure referrals move smoothly from first contact to patient transition.

What You’ll Do

• Develop and expand home health referral sources throughout Rockwall County and surrounding communities.
• Build relationships with physicians, hospitals, rehabilitation facilities, SNFs, ALFs, ILFs, house-call providers, specialty practices, and other healthcare partners.
• Identify and pursue qualified Traditional Medicare home health referral opportunities.
• Educate referral partners on home health services, Medicare eligibility requirements, skilled care needs, homebound criteria, and available services.
• Conduct regular face-to-face visits with existing and prospective referral partners.
• Generate consistent referrals and support monthly admission growth.
• Develop new referral accounts while strengthening existing relationships.
• Deliver educational presentations, in-services, and outreach activities.
• Participate in healthcare networking events and community partnership opportunities.
• Establish strategic partnerships with senior living communities and other high-potential referral sources.
• Maintain an organized referral pipeline and follow up consistently.
• Collaborate with intake and clinical leadership to facilitate timely referral coordination and smooth patient transitions.
• Track referral activity, admissions, account development, and territory growth.
• Meet established monthly business development and admission growth expectations.
• Represent the organization professionally while maintaining compliance with applicable healthcare marketing and referral regulations.

Requirements

• Home health industry experience strongly preferred.
• Minimum 2 years of healthcare marketing, liaison, outreach, or business development experience preferred.
• Established relationships with healthcare referral sources in Rockwall County and surrounding communities are highly desirable.
• Experience generating home health referrals and developing new accounts.
• Strong understanding of Traditional Medicare home health services, eligibility requirements, and referral processes.
• Demonstrated success in growing a healthcare territory or referral network.
• Comfort with cold outreach, physician office visits, facility visits, networking, and requesting referrals directly.
• Strong interpersonal, communication, negotiation, and presentation skills.
• Self-motivated, organized, dependable, and results-oriented.
• Ability to work independently and manage a field-based territory.
• Reliable transportation and willingness to travel throughout the assigned territory.
• Nursing or clinical licensure is not required.
• In your application, describe your experience developing new referral relationships, including cold calling, visiting physician offices or facilities without an appointment, following up with prospective referral partners, and converting those relationships into referrals or admissions.
