Job Description

OakMed Home Nursing & Rehabilitation is seeking a full-time RN Clinical Business Development Manager to coordinate hospital-to-home care transitions in the Grand Rapids area. Working onsite, this role partners with hospital and insurance case managers to drive growth in post-acute home health services.

Responsibilities

  • Support the care management team in transitioning patients from hospital systems to home care by reviewing the hospital census daily, nurturing existing relationships, and identifying correctly registered workers’ compensation and auto MVA patients to promote and secure growth.
  • Advocate for the company’s services and educate hospital staff, case managers, patients, and families about post-acute care options and the benefits of available programs.
  • Establish and maintain partnerships with hospital managers and act as the primary point of contact and relationship manager.
  • Review Michigan no fault benefits with patients and families to clarify auto policies, address inquiries to expedite services, assist with auto claims processes, and proactively resolve issues.
  • Ensure timely delivery of documents, physician orders, demographics, pre-admission materials, UD10 crash reports, insurance assessments, and assigned claims applications to secure necessary signatures and support reimbursement.
  • Develop a marketing plan by assessing needs, researching competitors, identifying expansion opportunities, and presenting and executing strategies to increase market share and meet company goals.
  • Record daily activity using quantitative methods and generateMonthly and quarterly reports to identify trends and overall program utilization.
  • Collaborate closely with the DON, ADON, nursing staff, and intake department to ensure admissions and address special patient needs while fostering teamwork and process improvement across care delivery.
  • Participate in trauma rounds weekly, deliver presentations, and coordinate events to raise hospital visibility, cultivate new relationships, and provide post-acute solutions.
  • Demonstrate a comprehensive understanding of post-acute processes, healthcare, and the home care and private duty sectors, with emphasis on auto insurance and workers’ compensation reimbursement.
  • Attend training sessions, conferences, and trade shows to enhance company visibility and stay informed on post-acute care practices and trends.
  • Other duties as assigned.

Requirements

  • Current Michigan RN in good standing.
  • Minimum 3-5 years of experience in home health care or related health care fields such as marketing, liaison, or case management, with a proven track record of meeting sales and budget goals.
  • Bachelor's degree in marketing, business, nursing or health management preferred.
  • Proven ability to lead, teach, and train others in post-acute options as well as generate leads, obtain qualified referrals, manage a territory, and maintain and build relationships with new and existing hospital-system customers.
  • Exceptional verbal and written communications, strong business acumen, and interpersonal skills.
  • Proficiency with Microsoft Office and platforms such as Outlook, Allscripts, and Epic.
  • Local travel required; must have a valid driver's license, reliable transportation, and auto insurance.
  • This position requires both time in the office and visits to local hospitals.
  • Hours are 8:30 AM to 5:00 PM, Monday through Friday.
