Job Description

Homewatch CareGivers of East St. Paul offers a field-based Business Development role built around real community connections across the East Metro. You will drive growth by strengthening referral relationships in senior care, with compensation that combines base pay and commission tied to clients who start and stay on service. The position is onsite in Lake Elmo, MN, with daily local driving across the territory and opportunities to participate in community events.

What you’ll do

  • Build and maintain referral relationships with hospitals, TCUs, skilled nursing facilities, home health and hospice agencies, assisted and independent living communities, and senior centers.
  • Establish trusted connections with decision influencers across the community, including discharge planners, social workers, case managers, physician offices, elder law attorneys, and financial planners.
  • Maintain a consistent weekly cadence of in-person visits, follow-ups, and educational drop-offs for priority referral partners.
  • Own referral relationships with East Metro hospital discharge teams, including Woodwinds Health Campus, M Health Fairview St. John's, and Regions.
  • Generate qualified client inquiries and convert them into signed service agreements and starts of care.
  • Handle hot referrals the same day and coordinate with the Owner and RN on in-home assessments.
  • Increase value per client by recommending the right level of care, not the minimum.
  • Support payer channels as they open, including VA Community Care, long-term care insurance, and employer backup-care partners.
  • Plan and deliver lunch-and-learns, in-services, and educational presentations for referral partners and senior audiences.
  • Represent the agency at networking events, health fairs, sponsored activities, and community events.
  • Log every visit, contact, and referral in CRM and the 52 Weeks Marketing system the same day, then report weekly on activity, referral sources, conversions, and starts of care.
  • Track which referral sources produce clients and adjust your effort accordingly.

What you bring

  • At least 1 year of sales, marketing, or business development experience in the senior care industry (home care, home health, hospice, assisted living, memory care, TCU/SNF, or a senior placement service).
  • A track record of generating referrals or clients with measurable results (for example, referrals per month, move-ins, admissions, or revenue).
  • Working knowledge of how hospital and facility discharges work and who influences home care decisions.
  • Comfort walking into facilities unannounced, requesting the decision-maker, and following up until a meeting is secured.
  • Strong verbal and written communication, including the ability to present to nurses or social workers.
  • Discipline with CRM and consistent weekly reporting.
  • Valid Minnesota driver’s license, reliable vehicle, and current auto insurance.
  • Must pass a Minnesota Department of Human Services background study.

Compensation and benefits

Pay combines base salary with commission tied to clients who actually start and stay on service.

  • Base salary: $55,000 to $65,000 per year (based on experience)
  • Commission: Tier-based un-capped commission, potential up to $60,000 per year for the right candidate
  • Cell phone reimbursement

  • Mileage: reimbursed at the IRS standard rate

Working conditions

  • Daily local driving across the territory; occasional evening or weekend events
  • Ability to stand, walk, and carry marketing materials up to 40 lbs
  • Occasional in-home visits with prospective clients and families

Preferred qualifications

  • Existing referral network in the East Metro (Woodbury, Maplewood, Stillwater, Cottage Grove, Eagan, Inver Grove Heights)
  • 2+ years in senior care sales, ideally with a private-pay home care agency
  • Familiarity with VA Community Care, long-term care insurance claims, or the Minnesota Elderly Waiver
  • Experience with employer backup-care networks or B2B partnerships
  • Bachelor’s degree in business, marketing, health administration, social work, or related field
  • Experience opening a new territory or growing a young agency
