Job Description

This field-based role drives referral development and census growth for home health services across Maine.

Responsibilities

  • Develop and sustain a comprehensive network of referral sources including physicians, hospitalists, discharge planners, case managers, social workers, SNFs, ALFs, and rehab centers throughout Maine.
  • Identify and qualify high-potential referral accounts, prioritize them by value, and design targeted strategies to win new business while expanding current relationships.
  • Conduct ongoing in-person prospecting, lunch-and-learn sessions, and community education presentations to raise visibility and drive referral volume for Interim HealthCare.
  • Respond promptly to new referrals and act as the liaison linking referrers with the clinical operations team.
  • Develop and implement a territory business plan with defined targets for new accounts and census growth in home health.
  • Prepare and present information on Medicare-certified home health, hospice, and personal care services.
  • Maintain a complete log of prospecting, account development, and referral activity in CRM systems (Salesforce or equivalent).
  • Provide weekly reports on pipeline activity, referral trends, and territory performance metrics.
  • Collaborate with clinical managers to monitor service quality and reassure referral sources about patient care.
  • Participate in community health events, healthcare association meetings, and networking functions.
  • Maintain current knowledge of Medicare home health eligibility, Maine state regulations, and competitive dynamics in the Maine market.

Requirements

  • At least three years of B2B sales experience in home health, hospice, medical, or broader healthcare services.
  • Proven ability to cultivate referral relationships with physicians, discharge planners, SNF/ALF staff, case managers, and social workers.
  • Knowledge of Medicare-certified home health services, eligibility criteria, and referral workflows.
  • Familiarity with Maine and federal home health standards and regulations.
  • Results-oriented communicator comfortable working independently in a field-based, territory role.
  • Experience with CRM platforms such as Salesforce or HubSpot for pipeline tracking and reporting.
  • Valid Maine driver's license and reliable transportation for regular territory travel.
